Community Rehabilitation Unit - How to be referred

Referrals for admission to CRU are accepted from:

  • The prospective patient’s GP
  • Members of the Primary Care Team (Use CRU referral form and form must be signed by prospective patient’s GP)
  • Consultants from the Medicine for the Elderly Department and Neurology Department, St. James’s Hospital
  • Staff Members from the Robert Mayne Day Hospital and Ortho-Geriatric Team, St. James’s Hospital
  • Members of the Home First Team, St. James’s Hospital
  • Members of the ICPOP (Integrated Care Programme for Older Persons) Team, CHO 7 (Cherry Orchard, St. James’s and Tallaght area)
  • Consultants from the Medicine for the Elderly Department; Tallaght Hospital
  • GEDI (Geriatric Emergency Department Intervention) Team, Tallaght Hospital

Please note that the prospective patient’s referral form must be signed by their own GP or Consultant Geriatrician.

Following referral, individuals will require a pre-assessment review with a Consultant Geriatrician and the CRU Clinical Nurse Specialist. This assessment will be organised by the CRU Clinical Nurse Specialist. Admissions to the CRU Service will be determined following a multidisciplinary team admissions meeting and final acceptance by the CRU Consultant Geriatrician
