This is the service provided by specialist nurses, doctors and a medical social worker who visit you, in your own home. It is most suited to people who are unable to attend one of our Out-patient services. The team do not assume the overall management of your care; your GP will always be your primary carer.
Yes. We offer you and your carer the choice to visit an Out-patient clinic at the hospice. The CPCT Out-patient clinic is the same service as that provided to people at home; the only difference is that you are coming into a clinic rather that a member of the team visiting you at home.
In our Out-patient clinics, we can review you at a flexible appointed time, that fits with your daily commitments (we understand that you might have a lot of other hospital appointments, demands on your time from family and friends, or dislike people coming into your own home). Attending the Out-patient clinic will reduce the need for home visits, and will also give you the opportunity to become familiar with the hospice, which may help reduce the anxiety often associated with hospice care. The flexibility of Out-patient review may provide quicker access to specialist advice and support.
